Granite Mountain Home Care & Hospice is a home health care agency, offering both medical and hospice home care to families in central Arizona. Our offices are located in Prescott AZ, and we also serve Anthem, Chino Valley, Cordes Lakes, Dewey, Mayer, Paulden, Peoples Valley, Prescott, Prescott Valley, Sedona, Skull Valley, Spring Valley, Wilhoit, Williamson and Yarnell, AZ and other communities throughout Yavapai County. Services include nursing care, physical and occupational therapy, post-hospital rehabilitation, extended care, wound care, home health aides, medical social workers, medication management, senior care and hospice care. Home Hospice caregivers provide pain and symptom control, palliative care, spiritual support, hygiene care, light housekeeping, grief and bereavement counseling, and physician oversight. We are an affiliate of Kindred at Home.
